I am the parent or legal guardian of the child(ren) being registered. I give permission for my child(ren) to participate in the church's Field Day event. I understand that participation involves normal physical activities and acknowledge there is a risk of injury. I voluntarily assume these risks and release the church, its staff, volunteers, and event organizers from liability for injuries or damages arising from participation, except where caused by gross negligence or willful misconduct. I also authorize event staff to obtain emergency medical treatment for my child(ren) if I cannot be reached. I certify that my child(ren) are physically able to participate and that I have provided any relevant medical or emergency contact information.

SAFETY
A member of Calvary's Safety Team will be onsite to help maintain a safe environment for this event. They will be clearly visible in bright shirts marked "SAFETY TEAM."

There will be a medical professional onsite to assist if any accidents or medical issues occur.
